Health Records and Immunizations

All incoming students (first year and transfer) are required to complete the following electronic forms on the Lake Forest Student Health Patient Portal: 

  • Immunization History Form – A signed/stamped copy of your immunization records must be uploaded into the portal for dates to be verified.  
  • Tuberculosis Questionnaire 
  • Medical History Form 
  • Consent to Treat Form (if under the age of 18 prior to the start of semester)  

All records submitted are confidential. When a student leaves the College, the record will be kept for no fewer than five years. Information will only be released with the written permission of the student.

Required Immunizations

Required Immunizations

A signed/stamped copy of your immunization records must be uploaded into the student health portal when completing the online Immunization History form.

In place of an original immunization record, you may submit our Immunization Record Form completed by your medical provider.

Upon arrival to campus, all students not in compliance with the immunization requirements will be restricted from registering for subsequent terms until compliance is obtained.

Exemptions

Medical Exemption Requests: Please ask your treating medical provider to write and sign a letter on official letterhead indicating the nature and probable duration of the medical condition(s) and/or circumstance(s) that contraindicate a vaccination(s).

Religious Exemption Requests: Please fill out the religious exemption certificate.

Submissions should be made as a PDF on the student health portal via “Document Upload.”

It is important to note that exemption from immunization may result in quarantine of non-immunized students off grounds in the event of a contagious disease outbreak. Lake Forest College reserves the right to deny non-immunized students access to campus or other College facilities in the case of an outbreak. The length of time a student is excluded from school will vary depending on the type of disease and the circumstances surrounding the outbreak and could range from several days to over a month.

Tuberculosis Questionnaire

All students must complete the Tuberculosis Questionnaire. A tuberculosis (TB) test is only required if a student responds “yes” to any of the questions on the questionnaire.

If a TB test is required:

  • Record TB test results using the Tuberculosis Testing Record.
  • Completed testing records can be submitted as a PDF on the student health portal via “Document Upload” or within the Tuberculosis Questionnaire form.
  • TB tests should be done no earlier than 6 months prior to arrival to campus and prior to registration for second semester.
  • It is recommended that students complete the TB test prior to arrival to campus, but the test may be done at the Health & Wellness Center at no cost to the student. Please email healthandwellness@lakeforest.edu to schedule an appointment.
Parent/Guardian Consent to Treat a Minor

We require Parental or Guardian Consent to treat any student who is under the age of 18.

The purpose of the consent is to allow for: 

1) The sharing/exchange of relevant medical information between Lake Forest College representatives (officials, faculty, staff), Lake Forest College Health and Wellness Center, and, for the purpose of diagnosis and/or treatment, other medical providers. Each of the above individuals or entities is also authorized to communicate and discuss health matters with the parents/guardians/emergency contacts of my minor child.

2) The transportation of my minor child, under appropriate circumstances, to area hospitals for diagnosis and treatment.

3) The provision, by Lake Forest College Health and Wellness Center, of such diagnostic, therapeutic, voluntary immunization, and operative procedures as may be deemed necessary for my minor child. The consent form can be uploaded to the Lake Forest Student Health Patient Portal.

Frequently Asked Questions

How do I get my immunization documents to Health Services?

You must upload your immunization records on the Student Health Portal. Under My Forms, you can complete the Immunization History Form. Within this form, you can upload copies of your immunization records. 

Will a hold be placed on my student account?

Yes, if you do not complete your immunization requirements, a registration hold will be placed on your student account. 

Can I get immunizations during New Student Orientation?

A local pharmacy hosts a one-time vaccination clinic on campus during New Student Orientation each year. If you are interested in receiving an outstanding immunization at the one-time vaccination clinic, please email our staff nurse, Josie Stams, at jstams@lakeforest.edu. 

How can I complete requirements once I arrive at Lake Forest College?

The Health and Wellness Center does not administer vaccines on campus. If you are missing a required vaccine and are unable to attend the one-time vaccine clinic during New Student Orientation, there are 3 pharmacies within walking distance of the college including Walgreens, CVS and Jewel-Osco. All 3 of these pharmacies offer the required immunizations. Same-day appointments can be made online. Please visit our Local Pharmacies and Hospitals page for pharmacy websites and directions.

If you need a tuberculosis test and are unable to get one prior to arrival on campus, you can get one at the Health and Wellness Center. If you need the Health and Wellness Center to administer a tuberculosis test, please email our staff nurse, Josie Stams, at jstams@lakeforest.edu.

Can my records be transferred from another school?

Records are not automatically transferred from one school to another.  You must obtain your immunization records from your other school and upload it through the Student Health Portal. Please be aware that different schools have different requirements.  Please see the immunization forms on this site to ensure you complete all requirements. 

How do I submit a waiver to be exempt from the student immunization requirements for a religious exemption or a medical exemption?

Can I submit my immunization records in any language?

No. All records must be submitted in English.  If you are from a country where English is not the language used on medical documents, we strongly recommend that you have your doctor complete ourimmunization form to ensure that you complete all the requirements. 

How do I meet the health requirements when I am an international student and can’t complete the requirements until I arrive in the United States?

International students should complete their immunization form and other health forms on the Student Health Portal. If you are missing required immunizations that are unavailable in your home country, we encourage you to sign up to receive the outstanding vaccine(s) at our one-time vaccine clinic during New Student Orientation. If you are required to get a tuberculosis test and are unable to complete the test before arrival to campus, the Health & Wellness Center also hosts a tuberculosis testing clinic during New Student Orientation. Is everyone required to submit a tuberculosis test?

No. Only students who answer “yes” to any of the questions on the Tuberculosis Questionnaire are required to submit proof of a tuberculosis test within the last 6 months. 

What happens if I do not upload my forms?

Health Services will not be able to verify your immunization data and a registration hold will be placed on your student account. 

I’m over 18 years of age. Do I need to complete the Parent/Guardian Consent to Treat a Minor Form?

No. Only students under 18 years old need to complete the Parent/Guardian Consent to Treat a Minor form.
