Applying for Accommodations

Step 1

Complete the Accessibility Services Application Form. Please complete the form in its entirety with clear and detailed information.

You may also submit 3rd party documentation with the form. Please see the documentation guidelines for a more comprehensive explanation of what documentation is appropriate.

Step 2

After completing the Application Form, students will receive information from an Accessibility Services Staff member to set up a Self-Report Meeting. Self-Report Meetings are approximately 45 minutes, during this meeting your documentation will be reviewed, and you will discuss the barriers you are facing. If it is determined reasonable accommodations need to be put in place, an accommodation plan will be developed.
